Make RAPI CreateInstance() arguments deployment-specific.
Add the GANETI_CREATEINSTANCE_KWARGS setting, which allows the administrator
to set arbitrary arguments to RAPI CreateInstance().

Also add initial version of README.Upgrade, detailing
upgrades of Synnefo deployments.
